FURFACTOR MOBILE DOG GROOMING POLICIES AND PET RELEASE FORM WAIVER
OUR MISSION: FurFactor Mobile Dog Grooming provides an stress-free mobile environment and has compassion for your concerns and your pet’s needs. Safety and comfort is our priority as well as providing regular companionship during the grooming process. If there is anything we can do to please you, or your pet better please let us know. We will not perform any procedures that may cause pain or any level of stress that we think is excessive. If we believe that your pet has high anxiety, overly stressed, or aggressive we may refuse services and or reschedule the appointment to ensure the safety of your pet and our groomers.
VACCINATIONS/MEDICAL SAFETY: We believe in keeping and maintaining a sanitized environment and grooming tools upon the completion of the grooming process. We strongly advise that each pet be vaccinated against hepatitis, parvovirus, corona leptospirosis, parainfluenza adenovirus type 2, Bordetella, and Rabies vaccinations prior to being groomed. You are responsible for providing up to date vaccination documents from your vet prior to any services. You must also advise our groomer of allergies, medical, physical, sensitivities, or any pre-existing conditions. This includes ear infections, skin problems, scars, moles, warts, and or hip or joint problems.
PHYSICAL/EMOTIONAL SAFETY OF YOUR PET & OUR GROOMERS: If your pet has been sedated, please let the groomer know before the grooming process. Grooming a sedated pet may be dangerous for the groomer and your pet. If your pet is a biter or snaps please make your groomer aware of this. In the event of the booked appointment and your pet is not able to be groomed due to aggression or any other issue, a $35.00 charge will be applied to your card. We use a safe muzzle for pets that show aggression. The pet owner will be responsible for any bites that must require medical attention/treatment and or any property damaged by their pet. All bites will be reported to the local authorities required by law. We will never force your pet to endure any uncomfortable procedure during the grooming process. During the grooming process we use very sharp instruments and pets may show impatient behaviors. We always take every precautionary measure, yet some situations are beyond our control. In the event of an accident during the grooming process we advise you to seek veterinary treatment at the owner’s expense. Due to insurance regulations we do not permit pet owners to stay in the mobile unit while their pet is being groomed.
FLEAS & TICKS: In the event of your pet having fleas or ticks there will be a $25.00 fee applied to your bill for flea removal and proper mobile unit sanitizing procedures. We use natural shampoo solutions to treat your pet for fleas or ticks. Some pets may be sensitive to these shampoos and may experience an allergic reaction. If we notice any problems, we will notify you immediately.
SKIN EARS & EYES: FurFactor Mobile Dog Grooming cannot be held liable for skin, eyes, or ear irritations or infections that may occur due to shampoo sensitivity. We clean ears with a professional pet ear cleaner. If there is an excessive amount of hair in the ear canal, it will be removed. This may result in rawness or ear splitting due to head shaking or any past irritations. We cannot be responsible for existing ear infections or any other emerging health issues. Trimming the hair around the eyes may create eye irritation due to hair particles entering the eye. We cannot be responsible for any self-inflicted injuries from scratching and or rubbing on any surfaces.
MATTED DOGS: In the event of your pet not being humanly de-matted, we may ask permission to clip the hair short for safety precautions. FurFactor Mobile Dog Grooming will not be responsible for any pre-existing conditions due to the matted coat. Removing a heavily matted coat includes the risk of cuts, abrasions, and raw irritated skin due to moles, warts, or skin folds hidden in the matted coat. FurFactor Mobile Dog Grooming will not be financially liable for any of these types of injuries created by the matted coat. The matting may result in an additional fee which is to be determined by the groomer.
PRICING: Pricing varies according to each pet and is determined by size, breed, the condition of your pet, and any additional services requested by the pet owner. Additional charges may apply that may be conducted to complete the grooming process. This may include de-matting, de-shedding, flea/tick treatments, and any additional time and materials it has taken to complete a safe grooming process for your pet.
CANCELLATIONS/NO SHOW: We require a 24 hour notice to cancel or change your booked appointment. Same day cancellations and or no shows will result in a $50.00 cancellation or no-show fee.
APPOINTMENT TIMES: All appointment times are estimated time frames. They are subject to traffic, weather, pet behaviors, and or any other unforeseen circumstances. We always strive for success to accommodate time restraints. Safety and Empathy of your pet is our mission and will never be compromised. We strive for quality in our service and care about building relationships with you and your pet.
SOCIAL MEDIA PHOTO RELEASE: I hereby allow/consent FurFactor Mobile Dog Grooming to advertise photos of my pet on Social Media or any Web Based Platform.
RELEASE: I am the owner of this/these pets(s). I have read and understand the above listed sections performed by FurFactor Mobile Dog Grooming. I consent and authorize/release my pet(s) to be groomed by FurFactor Mobile Dog Grooming and its employees from any responsibility or liability. I agree to pay the full amount due for services and completion of all services performed.
